Since ES's path from the old thread is gone forever, I'm stealing wuLFe's video and posting it here, which uses the same path.

http://youtube.com/watch?v=GFQXseDmX3E

The 1st activation can go one note further, but it's extremely tough, even for me. I usually don't go for it. I think the last activation can last to the yellow as well, but I dunno.

EDIT: It does.

EDIT 2: For those who prefer charts to videos, I rummaged through Piso's photobucket pics and stole his Round and Round chart: http://i144.photobucket.com/albums/r162/Piso_Crap/RoundAndRoundExpertPath.png

I just did 2-2-2-2-2 and it worked out pretty well. It's usually pretty obvious when is the right time to activate.

debr5836's upperbound chart

Looking at that, it doesn't look like it. That gets you 299,5xx with full tick squeezing, and that's with maxing out the early-whammy on every SP hold and fully abusing the timing window on squeezes. The only other squeezes that appear beyond that are two single-note squeezes, which appear likely to be impossible, and even with those you'd only be at 299,9xx. Close, but not quite.
