Mine's GH related, too.
Back when GH:A came out, I still haven't FC'd a single song on expert on any plastic guitar game, so I was trying to FC Dream Police. I was almost done with the song and I knew it. My whole body felt like it was on fire and my stomach really started to hurt because I was so nervous. But I got the FC and I was happy. Then the next time I tried to FC another song on expert and I didn't choke. It's pretty easy now to FC songs and I don't get nervous anymore.  _________________

Anyways to contribute, I was playing left 4 dead (Not really a horror game.) we were doing BH on expert and we had killed both of the tanks we jumped down and instantly boomer barfed one guy got smoked, and it was only me and this other a computer left, the computer left me to save the smoked guy and then everyone got zomb raped... I still haven't beat BH on expert yet despite 6+ hours total of trying to do BH5. Haven't even got to the second tank now in awhile.
When we all got barfed was pretty much the scariest thing for me ever. |

The final Gruntilda battle in Banio-Kazooie for the N64 had me shaking and sweating. That battle was tough for my 9-year-old self then, and my health was so low as I was trying to shoot eggs into the final Jinjo statue (I had already died numerous times). That's probably one of the most rewarding boss defeats I've ever experienced. _________________

This isn't really related to any game in particular, but whenever I fail a mission or something in a game, I always get a little scared to try it again for some reason. I don't know why. _________________

On Harry Potter 1 for the PS1, I would always freak out at the level where it's nighttime and you have to get to Hagrid's Hut. The aspect of it being nighttime frikin made it so scary for me. |

Not horror game huh..Well, on Fallout 3 i was wandering aimlessly through the wastes and came upon this town, There was this one monster thing outside called a deathclaw. It raped me and i just barely killed it. My friends were like wow you just got owned, then i went into the city thinking it was abandoned, and was like shit i need to heal (No stimpaks) so i went to fast travel and got this. "Cannot fast travel with enemies nearby" I was like what? And turned around and there were three deathclaws that attacked me right as i turned around. That scared the fuck out of me. _________________

Majora's Mask: I basically passed two temples when I turned off the N64, then I realized I had forgotten to save after those TWO temples. I was nearly scared shitless. Luckily, I had saved right after I defeated Twinrova. Man it was scary. |

LOL I remember reading that on the "Beating Games That Haunted Your Past" thread.
I'd say just about every time I think I didn't save in a game, but it turns out I really did is equally scary to me. Can't think of a specific time, however. _________________

Mine was when I was playing a fan-made version of doom2.
I walked around a corner to the exit and saw this giant pepper monster eating my face. It went down something like this

This series in general scares the shit out of me when I do good (nerves) and pisses me the fuck off when I do poorly: the feelings pretty much parallel Guitar Hero.
Close calls here were at 1:44 and 4:07. 1:44 I didn't care much about, but 4:07 nearly gave me a heart attack--I narrowly escaped death by probably no more than a pixel--causing the next 15 seconds to be a ridiculous adrenaline rush.

You can't even begin to fathom how nervous I was getting by the end of that run.
Anyone who has ever beaten I Wanna Be The Guy can probably tell you just how unbelievably stressful the last boss is to beat. As well as the OVERWHELMING joy that comes with finally succeeding. _________________
