I also noticed the same problem 2 more times on the same page maybe the rankings just get bugged once they reach the thousands place

They're not bugged any further in the latter ranks than in the top ranks. However, the odds of being detected and reported is probably far less in the lower ranks leading to this perception.
For purposes of maintaining good performance, all rankings are stored and updated (rather than calculated each time you view a page). While this drastically speeds things up, it has the downside that in the rare occasion an update fails to complete, a score can be inserted with the same rank as another (or as 0th) without incrementing the ranks of scores below it. Over time these missed updates can build up leading to multiple minor inconsistencies in the ranks.
It's a simple task to recalculate the ranks for a given song as these missed updates are detected, but there's no way to automate the process without slowing things down for everyone. That's why this thread exists so we can respond quickly and keep things as accurate as possible.

Two 5th places, no 21st place, two 23rd, no 24th, two 25th, no 28th, two 29th, no 31st, two 33rd. I didn't bother looking beyond place (-s) 33, pretty sure there are lots further down too.

This isn't really a ranking problem, but a top score problem. For some reason, the first person to achieve the 1st place score isn't always listed in Top Scores. For example, on Ace of Spades expert vocals, I am listed as first place even though, according to rankings, I was much later to achieve the score than others above me. In other words, I shouldn't be first for that song; NavyCherub should. It seems to just choose a random person from the tie  Conversely, I got Laid To Rest 95k first on GH:SH vocals, yet jerock is listed as first. Again, it seems to have just pulled a random person in 1st. I'm sure this isn't the only instance of the oddity... it always, however, occurs on a vocals song with a round score, i.e. no freeforms. I'm sure it doesn't exist in WT either, cause vox on that game are stupid.
List of songs with this problem:
Ace of Spades - GH:M Expert Vocals
Beautiful Mourning - GH:M Expert Vocals
Evil - GH:M Expert Vocals
Bark at the Moon - GH:SH Expert Vocals
Beast and the Harlot - GH:SH Expert Vocals

There are probably others, but those are all I've found.
